In an effort to maintain the best experience for our patients, we ask that you please review and acknowledge our Cancellation, No Show, and Late Arrival Policies for your future appointments.

We require a credit card on file and ask you to read and understand our policies and sign in compliance.

Cancellation/ No Show Policy:

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, if your appointment is not canceled at least 48 hours in advance or if you arrive 15 minutes late, please note the following policy will be applied:

 

Late Arrival Policy:

Meetings run late and traffic delays often occur, however, to maintain our schedule and prevent long wait times for other patients, we ask that you arrive at least 15 minutes before your appointment. If you arrive 15 minutes late, please note you will be charged as we reserved time for your visit.

We pride ourselves in providing excellent service for all our patients and appreciate your understanding for this policy. Should we ever have to call to cancel or reschedule your appointment within 48 hours, we’d like to provide you a credit of $100 for appointments with Dr. Nazarian and $50 for appointments with other providers. Please don’t hesitate to reach out to our staff members should you have any additional questions.
