Saline or Silicone

There are a number of decisions to be made on your breast augmentation journey. Dr. Nazarian and her team are there to guide you every step of the way. The first one is what type of breast implant: saline or silicone. Both are safe. In fact, the silicone gel breast implant is the most studied medical device in the world. Saline and silicone breast implants each have their pros and cons. The biggest advantage of silicone gel breast implants is that most patients think that they feel more natural. The downside is that if they rupture, which is extremely rare, it is not something that you will notice immediately.   The biggest advantage of saline implants is that they require a shorter incision, since they are filled with saline after placement. And if they leak, you will know it right away as your breast will deflate. The downside of saline is that if you are a woman without a lot of native breast tissue, you may run the risk of rippling. This is when ripples in the implant are visible through the skin or with certain movements or positions.

Fat Grafting to the Breast

There is also a third option when it comes to increasing breast volume — fat grafting to the breast. For patients who are:
  • worried about placing a foreign device in their body
  • only desire a slight bump up in volume
  • need some extra fat to provide upper pole fullness
Fat grafting to the breasts can be an ideal procedure. It is more or less two surgeries in one. First, Dr. Nazarian will remove fat via liposuction from an area of the body where you have an abundance. Typically, this is the abdomen or flanks. Second, she will take this fat, process it, and then inject the fat cells into the breast. The biggest benefit of fat grafting to the breast is that you are using your body’s own tissue. The drawback is that only about 50-70% of the injected fat cells will “take”. However, what does remain is there for good. In general, Dr. Nazarian will overfill the breast by about 20% to compensate for this fat cell die off. While fat grafting to the breast can be performed alone in those patients who only require a slight bump up in volume, Dr. Nazarian will often combine it with an implant to improve upper pole fullness or camouflage the edges of a breast implant in very slender patients.

Shape, Texture & Projection

Both saline and silicone gel breast implants come in different shapes. They can be round or shaped. The latter is often referred to as the “gummy bear” implant. It is most commonly used in breast reconstruction surgery as it provides the most structure. All breast implant types can also be either smooth or textured. In general, Dr. Nazarian uses a round, smooth breast implant, but all of the options will be discussed with you in detail during your initial consultation.  Breast implants also come in different profiles. This refers to the projection of the implant which is how high it would stand if lying flat on a table. A higher profile implant may be ideal, for example, for a patient who has a small frame but wants a larger implant.

Incisions & Placement

The last decision that must be made when it comes to your breast augmentation are where to place the incisions and where to place the implant. Proper incision placement is one of the key components to a successful breast augmentation outcome. To reduce any visible scars and achieve the most natural looking results, Dr. Nazarian typically places small incisions either beneath the breast in the natural crease or around the edge of the areola where they are less obvious.  Breast augmentation incision placement options are: 
  • Inframammary augmentation: incision underneath the breast (almost always preferred by Dr. Nazarian)
  • Periareolar augmentation: incision on lower half of the areola (higher infection and capsular contracture risk)
  • Axillary augmentation: incision placed in the armpit (higher bleeding and infection risk)
  • Umbilical augmentation: incision placed in the belly button (not performed very much anymore nationwide)
The right one for you is really going to depend on your individual anatomy, lifestyle and needs. The implant can also be placed either above or below the pectoralis muscle. Again, there are pros and cons to each. Typically, however, Dr. Nazarian will place the implant below the muscle as it has been shown to reduce the risk of complications such as capsular contracture and gives a more natural result in patients who don’t have a lot of their own breast tissue.

Asked Questions

If my breasts sag, do I need a breast augmentation or a breast lift?

It’s really going to depend on how much your breasts sag. In general, if you can place a pen underneath your breast and it doesn’t drop to the ground, you probably need a breast lift. Another indicator is nipple position. If your nipples sit below the crease of your breasts, you will probably need a lift.

I am worried about scars on my breast. Are they bad?

Scars are probably the single biggest deterrent for patients when it comes to any kind of surgical procedure. This is especially true with the breasts. They are such a vital part of our identity as women that it can be frightening to think about scars marring their beauty. But here’s the thing to remember with scars on the breast: they typically heal really well. Furthermore, I have a lot of experience with breast augmentations, and take the necessary time to make sure that any scars are as inconspicuous as possible. I hide them in the crease of the breast or around the nipple. The other thing with scars is that they always look pink at first, but they will get better with time. However, it can take up to a year so patience is important.

I keep reading about breast implant illness. Is this a real thing and should I be worried?

There is not a lot known about breast implant illness. It is really a catch-all term for a group of auto-immune symptoms that are thought to be tied to a woman’s breast implants. The most common symptoms are brain fog, fatigue, and joint pain. Breast implants are the most studied medical device in history, but we are still talking about a foreign device in the body. It is conceivable that some patient’s bodies might react to them. A few plastic surgeons dismiss breast implant illness out of hand. I do not. I think that if a patient has exhausted her medical options and found no underlying cause for her symptoms, and she wants her implants removed, I remove them. Period. My job is to be your ally, always.

What about ALCL? Can my implants give me cancer?

ALCL is extremely rare yet very real. The FDA as well as Canada Health are taking it very seriously. So far, all of the cases of ALCL, which is a very small percentage of the overall number of patients with breast implants, involve textured implants only. If you have textured implants, there is no need to panic. It does not mean that you will get ALCL. But you do want to pay attention, and if you get an unexplained fever then go to your plastic surgeon. When caught early, ALCL is totally curable. Really, the best way to stay on top of your breast implant health in general is to take advantage of your yearly follow up. I encourage all of my breast augmentation patients to come in and see me annually. It gives me the opportunity to check on you physically, and to discuss any breaking news that may be causing anxiety.

What size implant will give me a D cup?

It is impossible to say without assessing you in person. Also, cup size is a really poor indicator of your actual breast volume. This is because bra cup measurements vary widely between manufacturers. The better thing to focus on is the “look” you want to achieve. Once you have an idea, schedule a consultation to speak with a plastic surgeon. He or she can take your chest measurements and discuss the best options for delivering on your goals.

How do I prevent capsular contracture?

Capsular contracture is a hardening of the skin and tissues around the breast implant. It can be unsightly and painful. We now know that the risk of capsular contracture is directly linked to the breast and breast implant’s exposure to bacteria. So, the best way to prevent it is to see a really experienced breast surgeon who practices sterile technique. During the consultation, ask the surgeon about his or her capsular contracture rate as well as what is done if a patient gets it. Personally, my rate is really low (I have only ever had one patient who developed capsular contracture). But in the rare instance where a patient does get capsular contracture, I replace her implants without professional charges in the first year (OR and anesthesia fees would apply, however). Some breast implant companies warranty their breast implants against rupture and capsular contracture — they replace the implants and pay for part of the surgery as well.
